Every November, Concordia joins organizations across the country in recognizing National Home Health, Home Care, Palliative Care and Hospice Month – a time set aside to honor those whose calling leads them into the homes, lives and stories of the people they serve. At Concordia, we celebrate this as Home and Community Services Month, embracing the full spectrum of care provided by our home health, home care, palliative, hospice and supportive service teams.

This month gives us a dedicated moment to pause and give thanks for all you do.
But the truth is: your impact is felt every single day.

You bring skilled, compassionate care to patients in moments of healing, moments of hardship, and moments of transition. You support families through uncertainty. You create comfort, dignity and calm in the midst of illness. You advocate, you listen, you teach, you lift spirits – and  you do it all with grace.

Some days you travel many miles; other days, the distance you cover is measured in the emotional space you help someone navigate. Whether you are on the road, on the phone, at a kitchen table, at a bedside, behind a desk, processing referrals, scheduling visits, offering spiritual care, managing compliance, coordinating equipment, providing clinical guidance or supporting our agencies operationally – you are part of a team whose reach extends far beyond what any statistic can capture.

This year, we wanted to do something a little different to celebrate you. Our leadership team worked together to create “The Gratitude Recipe” – a lighthearted video tribute that reflects the many ingredients you bring to Concordia’s mission. We hope it brings a smile to your face and reminds you how deeply you are appreciated:
